TAMS-AC Shape the Future of Your Health Service Survey Prize Draw
1. Promotion Details
The Shape the Future of Your Health Service Survey is being conducted by Tamworth Aboriginal Medical Service - Aboriginal Corporation (TAMS-AC) to gather feedback from clients and community members to help shape the future of health and wellbeing services across the organisation's service footprint.
The survey is open to people connected with TAMS-AC across the Tamworth, Gunnedah and Liverpool Plains Local Government Areas, including clients, program participants, families and community members.
Eligible participants who complete the survey and provide their contact details will be entered into the prize draw.
2. Eligibility
To enter the prize draw, participants must:
- Be aged 18 years or over.
- Complete the Shape the Future of Your Health Service Survey.
- Provide valid contact details at the end of the survey.
- Live in, access services in, or be connected with the TAMS-AC service footprint across the Tamworth, Gunnedah and Liverpool Plains Local Government Areas.
Participants do not need to be current TAMS-AC clients to complete the survey or enter the prize draw.
Only one survey entry per person will be accepted.
Employees directly involved in administering the survey or prize draw are not eligible to enter.
3. Survey Period
- Survey opens: Upon publication.
- Survey closes: 11.59pm Sunday 23 August 2026.
- Entries received after this date and time may not be included in the prize draw.
4. Prizes
Five prizes are available to be won:
- One (1) Adult Pushbike and Helmet Package
- One (1) Set of Wireless Noise-Cancelling Headphones
- Three (3) Fruit and Veggie Packs
Prizes are not transferable, exchangeable or redeemable for cash.
If a prize becomes unavailable, TAMS-AC reserves the right to substitute it with a prize of equal or greater value.
5. Prize Draw
The prize draw will be conducted on Wednesday 26 August 2026.
Winners will be selected at random from all eligible entries received during the survey period.

The result of the draw is final and no correspondence will be entered into regarding the outcome.
6. Winner Notification
Winners will be contacted by phone using the contact details provided in the survey.
If a winner cannot be contacted after reasonable attempts over a 14-day period, TAMS-AC reserves the right to conduct a redraw.
7. Privacy, Data Sovereignty and Use of Information
At TAMS-AC, your privacy and data sovereignty matter.
We are committed to ensuring that information shared by community is collected, stored, used and reported respectfully, securely and responsibly.
The survey has been designed to allow participants to provide feedback anonymously. Contact details collected for the prize draw will be used solely for prize administration purposes and will not be publicly shared.
Survey responses will be used to:
- Understand community priorities and needs.
- Improve existing services.
- Support future planning and service development.
- Strengthen connections with clients and community.
- Inform community engagement and quality improvement activities.
Survey findings may be shared publicly in a combined and de-identified format through:
- Social media posts.
- Community flyers.
- Reports and presentations.
- Community engagement activities.
- Other TAMS-AC communications.
Individual responses will not be publicly identified.
Because survey responses are anonymous, TAMS-AC is unable to identify, locate or remove a specific survey response once it has been submitted.

8. Storage and Security of Information
Survey information will be securely stored by TAMS-AC at:

Access to survey information will be restricted to authorised TAMS-AC staff involved in survey administration, analysis and reporting.
TAMS-AC will take reasonable steps to protect information from unauthorised access, misuse, loss or disclosure.
9. Consent
By completing the survey, participants:
- Consent to their survey responses being used for service planning, community engagement and continuous improvement purposes.
- Acknowledge that survey findings may be reported in a combined and de-identified format.
- Confirm that they understand anonymous responses cannot be individually identified or removed once submitted.
- Agree to these Terms and Conditions.
