Integrated Team Care (ITC) Program
TAMS-AC is proud to now deliver the Integrated Team Care (ITC) Program for our community.
The ITC Program helps Aboriginal and Torres Strait Islander people living with chronic health conditions to access the care they need, including support with:
Specialist appointments
Transport
Medication reviews
Care coordination
Cultural support
Who delivers ITC?
If you’re a TAMS-AC GP patient, your ITC services will now be delivered by TAMS-AC.
If you see another GP, HealthWISE will continue to support your care.
Eligibility
You may be eligible if you are Aboriginal or Torres Strait Islander and have a diagnosed chronic condition. Ask your GP for a referral.
